Ordinary email is not a secure or confidential channel. Send enough to explain the question. Save the detail for the call.
The form asks a few questions based on what you are writing about. It is short. Answer what you can and leave the rest.
New York Public Health Law Section 574 blocks kits and samples going to or coming from New York. I cannot mail you a kit there and I cannot accept a sample collected there. Residents work through a practitioner or collect in a neighboring state.
